A Practical Checklist for Measuring Blood Pressure at Home

A repeatable routine makes a blood pressure log easier to review. Use this checklist alongside your monitor's instructions and the schedule agreed with your healthcare professional.

Before you start

  • Avoid smoking, caffeine and exercise for at least 30 minutes beforehand.
  • Empty your bladder and rest quietly for at least five minutes.
  • Prepare the monitor, correctly sized cuff and a place to record the result.

Get into position

  • Sit with your back supported, feet flat and legs uncrossed.
  • Place the cuff on bare skin, following the placement marks in the manual.
  • Support your arm so the cuff is at heart level.
  • Stay still and do not talk during the measurement.

For preparation and positioning, see the American Heart Association home measurement guidance.

Measure and keep the actual results

Agree the timing and number of readings with your care team. General guidance recommends repeated readings, but the interval in your device instructions may differ. Follow the plan your clinician has confirmed for your monitor.

Write down both SYS and DIA, pulse if displayed, and the date and time. If your monitor has separate user memories, check the selected user before starting. Keep an error message separate from a completed reading.

Use this quick log format: Date / Time / User / SYS / DIA / Pulse / Notes. In “Notes”, record anything you want to discuss, such as movement during the measurement or an error symbol. Do not replace the real result with a target number.

Bring useful information to your appointment

Take the log and your monitor when practical. Ask your healthcare professional to check your technique and explain which results require action. Home monitoring should support your care, not become a reason to change medicine independently.
